# A GLP-1 symptom diary for four weeks.

A free, printable GLP-1 symptom diary. Each week is a grid: one column a day, one row for each of twelve common side effects, plus how the day felt, appetite and food noise. Mark each symptom mild, moderate or severe. Four weeks fit on two pages. It comes from Max HP, a GLP-1 dose tracker for iPhone.

## How to fill it in

- **Dose taken?**: Fill in the circle on dose day, so each week lines up with your dose.

- **Overall**: G, O or R: great, off or rough.

- **Symptoms**: Leave blank for none, or write 1 for mild, 2 for moderate, 3 for severe.

- **Appetite**: L, U or M: less, usual or more.

- **Food noise**: Q, S or L: quiet, some or loud. The [food noise log](https://maxhp.me/glp-1-food-noise-log) has room for notes.

A blank day means you didn’t write anything, not that you felt fine. Mark “Overall” every day, even when nothing happened, and a quiet week will read as a quiet week.

## The twelve symptoms

Nausea, constipation, diarrhea, heartburn, bloating, vomiting, fatigue, headache, dizziness, stomach pain, hair shedding and injection-site reactions, with a row for anything else. They’re the same names Max HP uses in its check-in, so a paper diary and the app read the same way.

The list is for recording, not a list of what to expect. Your medication’s label lists its side effects, and your prescriber or pharmacist can tell you which ones to report and when. If something is severe or worries you, contact them; don’t wait for the next appointment.

## Why by dose step

Side effects often change after a dose change. Keep one diary for each dose, and at the appointment two diaries side by side show how this dose compares with the last. Max HP does this comparison for you, in a [Step Review](https://maxhp.me/dose-steps) and on the [Clinic Report](https://maxhp.me/clinic-report), which has the same grid.

## Questions

**What’s the best way to track GLP-1 side effects?**

Daily, briefly and in the same format: which symptom, how strong, and on which day of the dose week. Recording how long each one lasted makes it easier to compare one dose with the next.

**Is the symptom diary free?**

Yes. Print it, copy it and share it. No email address needed.

**Does the diary tell me whether a symptom is normal?**

No. It’s a record for you and your prescriber. Ask them about anything you notice.
