A GLP‑1 appointment checklist: what to bring and what to ask.
A one‑page checklist for a GLP‑1 appointment: the nine things worth knowing before you go, five questions you might ask, and room to write down what you and your prescriber decide. It’s free to print. It comes from Max HP, a GLP‑1 dose tracker for iPhone.
Bring or know
- Your medication, its form (pen, vial or pill) and current dose
- The date each dose started
- Doses missed, late or taken on a different day
- Side effects at each dose: which, how strong, how many days
- Your weight trend since the last appointment
- Injection-site reactions, and at which sites
- Supply or pharmacy problems
- Other medications and supplements, and any changes
- Anything new: other conditions, surgery coming up, plans to become pregnant
Most of these come down to one question your prescriber is likely to ask: how did the last dose go? Answering it from memory is hard. A dose log and a symptom diary make it easier.
Questions you might ask
- How is this dose working for me, from what you see?
- What do I watch for, and when do I call?
- What’s the plan for my dose from here?
- Is there anything I’m doing that I could do differently?
- When is my next appointment, and what will we look at then?
These are open questions on purpose. They leave the medical decisions with your prescriber, and they tend to get a fuller answer than yes or no.
What we decided
The bottom of the sheet has room for what you agreed: the dose from here, as prescribed, anything to watch for, and the date of the next appointment. Writing it down at the time is easier than remembering it later.
The checklist, already filled in
If you log in Max HP, the Clinic Report answers most of the first list on one page: each dose step with its dates, doses on time, weight change and side‑effect days, plus notes you wrote for your prescriber. Print it, or share the PDF from your iPhone. Add the appointment date and Max HP reminds you to prepare it a few days before, in time to send ahead. See a sample report.
Questions
- What do I bring to a GLP‑1 appointment?
- Your current dose and when each dose started, any missed or late doses, side effects with how strong and how long, your weight trend, injection-site reactions, supply problems, and changes to other medications.
- What questions can I ask my prescriber about GLP‑1 medication?
- Open questions work well: how this dose is working from what they see, what to watch for and when to call, the plan for your dose from here, and what you’ll look at next time.
- Is the checklist free?
- Yes. Print it, copy it and share it. No email address needed.
